E-Rate Step 4: Form 471
Form 471: Services Ordered and Certification Form is filed to officially request discounts on eligible services to be provided to eligible entities. It specifies what you would like to purchase, from whom, and how much it will cost.
Calculate the Discount Level: Form 471 helps you calculate the discount by listing the recipients of services for support. FCC rules include a discount matrix that takes into consideration poverty level and the urban or rural location of your schools.
Submit Form 471: Form 471 must be submitted online or on paper and be certified by an authorized person to be considered complete. It must also be postmarked or submitted online prior to the close of the application filing window for the funding year to be considered as filed within the window.
When to File: Early November to early February preceding the start of the Funding Year (exact dates for each funding year will be posted on USAC's web site). Received or postmarked no later than 11:59 p.m. EST on the day of the close of the Form 471 application filing window.
